You are an excellent candidate for a neck lift if you are bothered by fullness under the chin or a double chin. These concerns can make you look older or heavier than you are, and they are often difficult to address with diet and exercise alone. The procedure is ideal for patients with moderate-to-significant aging in the neck and jawline. Patients under the age of 40 typically have adequate skin elasticity to remove fat under the chin and let the skin shrink to fit the new contour. In patients with less elasticity — older patients or those with a history of significant weight loss — the contour can be improved, but excess skin must be removed via facelift or direct neck-lift incisions. A neck lift is often performed on its own but is frequently combined with a facelift or chin liposuction for a more comprehensive lower-face rejuvenation.

What Do Scars Look Like?

A neck lift requires incisions, placed in inconspicuous locations. The primary incision is typically made under the chin (3–4 cm), with additional small incisions around the ears in some cases — all well-concealed once healed. With proper aftercare, these incisions heal into minimal, fine-line scars that continue to fade. The long-lasting transformation of a neck lift is well worth the minimal, well-hidden scars.

A neck lift is an outpatient procedure performed under general anesthesia. The surgeon makes discreet incisions under the chin and sometimes around the ears. Through these incisions, the surgeon tightens the underlying platysma muscle, removes excess fatty tissue deep to the platysma (a “deep neck lift”), and may use liposuction to refine the contour.

The neck-lift conversation is really about your skin elasticity

Almost every consultation for a neck lift comes down to one question: how elastic is your skin? Patients in their late 30s and early 40s often have the right balance — fat under the chin to remove, with skin elastic enough to retract smoothly to the new contour after liposuction. That's a clean neck-lift candidate. Patients in their late 50s and beyond, or anyone with a history of significant weight loss, usually need more than liposuction — the skin doesn't retract on its own, so we need to plan a deep-neck-lift incision (under the chin) or a full facelift incision (around the ears) to remove the excess. Getting this judgment right at consultation prevents the worst outcome: a flat neck with hanging skin draping over it. We plan the surgery to your tissue, not to a protocol.

You will have swelling and bruising in your neck and face. You will have a drain that is typically removed day 1 after surgery. A compression garment will be worn. Pain is minimal and can be managed with tylenol and ibuprofen in most cases.

Most of the swelling and bruising will have subsided. You can typically return to work and light, non-strenuous activities.

3 – 4 Weeks

The remaining swelling continues to resolve, and your final results begin to take shape. You can return to a full exercise routine.

All residual swelling will have completely resolved, and the final results of your neck lift will be fully apparent and stable.
